Applications

Applications to the program are done through a referral from a Community Justice Worker. The referred individual is then required to contact program staff to schedule an assessment time. To obtain a referral form, the Community Justice Worker can call or email Diane Petrofski at St. Leonard’s Society of Toronto (SLST).

Referral forms can be faxed to SLST.
